An 18-digit reference number can be generated as a result of clicking the "Save for later" button. The IMDb staff will be able to view the saved submission form. Consider posting that, please.
What you have posted is a picture of the problem. It's noted in the writer's guide that you cannot change WGA locked credits. If you have a screenshot of the actual credits which are different than what is listed on the site, an employee will happily help you out.
As mentioned by Adrian, please provide a screen shot of the end credits where the listings are different from those listed on the site. We can review these and make any changes applicable.
