Adding Myself To Movie Credits

So, I was an extra in the upcoming film "The Fault in Our Stars" where I was a patient in a hospital scene. I tried to add my credit to the cast list a few months ago and I was just wondering how can I prove to IMDB that I was an extra?
I have a pay stub and my tax return from 20th Century Fox, I was just wondering who I could contact to actually get my credit on IMDB.

1) Does your name appear in the on-screen credits of the show, if yes then can you obtain a screenshot of those credits. 2) If your name does not appear in the on- screen credits then you can submit your name as "uncredited" if you can be clearly seen and identifiable on screen and IF you already have a credited appearance listed on IMDb. 3) If you do not already have an on-screen role credited on IMDb, you are not yet eligible for an "uncredited" listing.
